LIABILITY WAIVER

All participants must watch our orientation video and sign a Spire Climbing Center Waiver.  Participants under 18 years of age MUST have waivers signed by a parent or court-appointed guardian.  If you have never visited us before or it’s been more than one year since you completed a waiver, you will need to complete a new waiver.  Save time by completing your waiver online before you arrive, and we'll have it on file when you get here! 

A government issued Photo ID is required for validation of waivers.  Please bring your ID with you upon your visit. 

INSTRUCTIONS FOR PARENTS:

• ADULT NAME -and- ADULT BIRTHDAY must be entered first.  Childs information will be entered later.
• Waivers for minors (under 18) must be signed by a PARENT (or COURT APPOINTED legal guardian).
• Waivers signed by a friend, relative or group leader WILL NOT BE ACCEPTED.
• If you are not visiting the facility with your minor, you MUST send your child or supervising adult with a photocopy of your picture ID.  Your child will not be able to participate without verification of your ID.
